Optimism linked to lower stroke severity, inflammation

Stroke survivors with high levels of optimism had lower inflammation levels, reduced stroke severity and less physical disability after three months, compared to those who are less optimistic, according to preliminary research presented at the Nursing Symposium of the American Stroke Association's International Stroke Conference 2020 -- Feb. 18-21 in Los Angeles.
In a small study of 49 stroke survivors, researchers examined the relationship among optimism, inflammation, stroke severity and physical disability for three months after a stroke. Researchers said that understanding how these elements relate to or impact one another may provide a scientific framework to develop new strategies for stroke recovery.
"Our results suggest that optimistic people have a better disease outcome, thus boosting morale may be an ideal way to improve mental health and recovery after a stroke," said Yun-Ju Lai, Ph.D., M.S., R.N., the study's first author and a postdoctoral fellow in the neurology department at The University of Texas Health Science Center at Houston.
Post-stroke inflammation is detrimental to the brain and impairs recovery. Optimism has been associated with lower inflammation levels and improved health outcomes among people with medical conditions, however, no prior studies assessed if this association exists among stroke patients.
This pilot study is a secondary analysis of data collected from a repository of neurological diseases. Outcomes included optimism levels from the revised Life Orientation Test, a standard psychological tool for measuring optimism; stroke severity evaluation through the National Institutes of Health Stroke Scale, and levels of inflammatory markers -- interleukin-6 (IL-6), tumor necrosis factor alpha (TNF?) and C-reactive protein (CRP).
As optimism levels increased, stroke severity and the inflammatory markers IL-6 and CRP decreased even after considering other possible variables. However, this was not true of TNF?.
"Patients and their families should know the importance of a positive environment that could benefit the patient," Lai said. "Mental health does affect recovery after a stroke."

A happy partner leads to a healthier future

Michigan State University research found that those who are optimistic contribute to the health of their partners, staving off the risk factors leading to Alzheimer's disease, dementia and cognitive decline as they grow old together.
"We spend a lot of time with our partners," said William Chopik, assistant professor of psychology and co-author of the study. "They might encourage us to exercise, eat healthier or remind us to take our medicine. When your partner is optimistic and healthy, it can translate to similar outcomes in your own life. You actually do experience a rosier future by living longer and staving off cognitive illnesses."
An optimistic partner may encourage eating a salad or work out together to develop healthier lifestyles. For example, if you quit smoking or start exercising, your partner is close to following suit within a few weeks and months.
"We found that when you look at the risk factors for what predicts things like Alzheimer's disease or dementia, a lot of them are things like living a healthy lifestyle," Chopik said. "Maintaining a healthy weight and physical activity are large predictors. There are some physiological markers as well. It looks like people who are married to optimists tend to score better on all of those metrics."
The study, published in the Journal of Personality and co-authored by MSU graduate student Jeewon Oh and Eric Kim, a research scientist in the Department of Social and Behavioral Sciences at the Harvard T.H. Chan School of Public Health, followed nearly 4,500 heterosexual couples from the Health and Retirement Study for up to eight years. The researchers found a potential link between being married to an optimistic person and preventing the onset of cognitive decline, thanks to a healthier environment at home.
"There's a sense where optimists lead by example, and their partners follow their lead," Chopik said. "While there's some research on people being jealous of their partner's good qualities or on having bad reactions to someone trying to control you, it is balanced with other research that shows being optimistic is associated with perceiving your relationship in a positive light."
The research also indicated that when couples recall shared experiences together, richer details from the memories emerge. A recent example, Chopik explained, was Google's tearjerker Super Bowl ad, "Loretta," in which an elderly man uses his Google Assistant to help him remember details about his late wife.
"The things he was recollecting were positive things about his partner," Chopik said. "There is science behind the Google ad. Part of the types of memories being recalled were positive aspects of their relationship and personalities."
With all of its benefits, is optimism something that can be prescribed? While there is a heritable component to optimism, Chopik says there is some evidence to suggest that it's a trainable quality.
"There are studies that show people have the power to change their personalities, as long as they engage in things that make them change," Chopik said. "Part of it is wanting to change. There are also intervention programs that suggest you can build up optimism."
Across the board, everyone benefits from a healthy dose of optimism from their partner. For the glass-is-half-empty people, a partner can still quench their thirst. For the glass-is-half-full people? Their cup runneth over.

However, data on the effect of weight and Body Mass Index on physical and mental health are rare -- especially among college students, who suffer high rates of anxiety and depression and often neglect physical self-care and exercise.
To that end, researchers from the University of Michigan and Fudan University in China set out to learn the extent to which BMI and positive outlook affect the physical and mental health of college students in China's Fudan University.
They found that a positive outlook and BMI both contributed significantly to good health, said Weiyun Chen, associate professor of health and fitness at the U-M School of Kinesiology.
Researchers asked 925 students to rate four indicators of psychological well-being: hope, gratitude, life satisfaction and subjective happiness. They also calculated students' BMI based on self-reported body weight and height. To assess physical and mental health, researchers asked students various questions about their sleep quality and how often they felt healthy, energized, worthless, fidgety, anxious or depressed.
Chen said that taken together, the four psychological variables and BMI accounted for 41 percent of the total variance in health. Individually, subjective happiness had the most significant impact, followed by hope, and then BMI.
By themselves, gratitude and life satisfaction didn't influence overall health. Also, interestingly, BMI was correlated with physical and overall health, but not with hope, gratitude, life satisfaction or mental health.
In light of the intense academic pressure Chinese college students face, especially at elite institutions like Fudan, Chen said she was surprised by how many students rated themselves happy and healthy. This could point to China's emphasis on well-being in schools.
"They have structured, organized physical educations classes," Chen said. "It's not just fitness, it's a variety of things so you can meet different people's needs. They realized that emphasizing only academics isn't good for overall health, and that they needed to emphasize the wellness part."
These numbers might look different for college students in the U.S., where two of three adults are overweight or obese, and 17 percent of youth ages 2-19 are considered obese, according to the CDC.
By contrast, 714 Fudan students, or 77.2 percent, were classified as normal body weight, while only 83 students were overweight, and just 5 students were obese, with 123 students considered underweight.
"Over the past 20 years, the United States has shrunk physical education in elementary school and in college," Chen said. "In China, especially in the past decade, they have started to emphasize physical education, and they are taking a holistic, whole person approach."
Chen said the findings suggest that universities should creatively design wellness programs and centers that dynamically integrate body, mind and spirit into a seamless unit.
The study has several limitations: all students were recruited from one university, and the results cannot be generalized; the research design prevented establishing causal effects; and the study did not account for gender differences.
