'Loss of pleasure' found in teen sleep study

But easy interventions can improve mental health

Sleep patterns around the world have been disrupted as screen time increases and sleep routines change with COVID-19 self-isolation requirements.

Negative mood is not unusual in adolescence, but lack of sleep can affect mental health, causing anhedonia (or loss of pleasure), anxiety, anger and significantly increasing the risk of depression, a global study of more than 350,000 teens shows.

The results just published in Sleep Medicine Reviews connects less sleep with a 55% increased chance of mood deficits and double the risk of reduced positive mood.

From Asia, to Australia, New Zealand, Europe and North America, sleep clearly was a modifiable risk factor that can improve or depress mood in adolescents, says Flinders University sleep researcher Dr Michelle Short.

"Sleep duration significantly predicts mood deficits on all mood states, including increased depression, anxiety, anger, negative affect and reduced positive affect," she says, with less sleep linked to an 83% higher chance or anger, 62% increased risk of depressed mood, and 41% higher risk of anxiety.

"Fortunately, there are many interventions individuals, family, the community and even public policy can encourage to maintain regular sleep in this at-risk population to reduce the likelihood of these problems spilling over into mental health issues needing clinical treatment," she says.

The researchers also recommend increased parental / guardian regulation of sleep and technology use, delayed school starting times, and monitoring academic and other pressures such as out-of-hours tutoring does not impede sleep routine.

Dr Short says that "while positive mood doesn't get much attention, it is still clinically relevant as one of the key symptoms of depression in anhedonia (loss of pleasure)."

"It is imperative that greater focus is given to sleep as for prevention and early intervention for mood deficits," the study concludes.

Insufficient sleep associated with risky behavior in teens

Mental health issues, substance abuse, accidents more likely for high school students sleeping less than six hours per night

Researchers examined a national data sample of risk-taking behaviors and sleep duration self-reported by high school students over eight years and found an association between sleep duration and personal safety risk-taking actions.

 

Adolescents require 8-10 hours of sleep at night for optimal health, according to sleep experts, yet more than 70 percent of high school students get less than that. Previous studies have demonstrated that insufficient sleep in youth can result in learning difficulties, impaired judgement, and risk of adverse health behaviors. In a new study, researchers at Brigham and Women's Hospital examined a national data sample of risk-taking behaviors and sleep duration self-reported by high school students over eight years and found an association between sleep duration and personal safety risk-taking actions. Results are published in a JAMA Pediatrics research letter on October 1.

 

"We found the odds of unsafe behavior by high school students increased significantly with fewer hours of sleep," said lead author Mathew Weaver, PhD, research fellow, Division of Sleep and Circadian Disorders, Brigham and Women's Hospital. "Personal risk-taking behaviors are common precursors to accidents and suicides, which are the leading causes of death among teens and have important implications for the health and safety of high school students nationally."

 

Compared to students who reported sleeping eight hours at night, high school students who slept less than six hours were twice as likely to self-report using alcohol, tobacco, marijuana or other drugs, and driving after drinking alcohol. They were also nearly twice as likely to report carrying a weapon or being in a fight. Researchers found the strongest associations were related to mood and self- harm. Those who slept less than six hours were more than three times as likely to consider or attempt suicide, and four times as likely to attempt suicide, resulting in treatment. Only 30 percent of the students in the study reported averaging more than eight hours of sleep on school nights.

 

The Youth Risk Behavior Surveys are administered biannually by the U.S. Centers for Disease Control and Prevention (CDC) at public and private schools across the country. Researchers used data from 67,615 high school students collected between 2007 and 2015. Personal safety risk-taking behaviors were examined individually and as composite categories. All analyses were weighted to account for the complex survey design and controlled for age, sex, race, and year of survey in mathematical models to test the association between sleep duration and each outcome of interest.

 

"Insufficient sleep in youth raises multiple public health concerns, including mental health, substance abuse, and motor vehicle crashes," said senior author Elizabeth Klerman, MD, PhD, director of the Analytic Modeling Unit, Division of Sleep and Circadian Disorders, Brigham and Women's Hospital. "More research is needed to determine the specific relationships between sleep and personal safety risk-taking behaviors. We should support efforts to promote healthy sleep habits and decrease barriers to sufficient sleep in this vulnerable population."

Sleeping too much or not enough may have bad effects on health

Fewer than six and more than ten hours of sleep per day are associated with metabolic syndrome and its individual components, according to a new study.

 

Researchers at Seoul National University College of Medicine found that compared to individuals who slept six to seven hours per day, men who slept fewer than six hours were more likely to have metabolic syndrome and higher waist circumference. Women who slept fewer than six hours were more likely to have higher waist circumference. Sleeping more than ten hours per day was associated with metabolic syndrome and increased levels of triglycerides in men, and with metabolic syndrome, higher waist circumference, higher levels of triglycerides and blood sugar, as well as low levels of 'good' cholesterol (HDL-C) in women. The authors found that nearly 11% of men and 13% of women slept less than six hours, while 1.5% of men and 1.7% of women slept more than ten hours.

 

Claire E. Kim, lead author of the study said: "This is the largest study examining a dose-response association between sleep duration and metabolic syndrome and its components separately for men and women. Because we were able to expand the sample of our previous study, we were able to detect associations between sleep and metabolic syndrome that were unnoticed before. We observed a potential gender difference between sleep duration and metabolic syndrome, with an association between metabolic syndrome and long sleep in women and metabolic syndrome and short sleep in men."

 

Based on common definitions, participants were considered to have metabolic syndrome if they showed at least three of the following: elevated waist circumference, high triglyceride levels, low levels of 'good' cholesterol, hypertension, and high fasting blood sugar. The prevalence of metabolic syndrome was just over 29% in men and 24.5% in women. The authors suggest that as the prevalence of metabolic syndrome in Korea is high, it is critical to identify modifiable risk factors such as sleep duration.

 

The authors used data from the HEXA study, a large-scale community-based study conducted in Korea during the years 2004-2013, which included information on socio-demographic characteristics, medical history, medication use, family history, lifestyle factors, diet, physical activity, and reproductive factors for women. As part of the HEXA study, samples of plasma, serum, buffy coat, blood cells, genomic DNA, and urine were collected, and participants underwent physical examinations by medical professionals. Sleep duration was assessed by asking the question: "In the past year, on average, how many hours/minutes of sleep (including daytime naps) did you take per day?"

 

Although the biological mechanisms that underlie the association between sleep duration and metabolic syndrome remain unclear, several potential processes have been reported. These include elevated levels of hormones which increase appetite and caloric intake or reduce energy expenditure in people who sleep less than seven hours per day, which may lead to increased waist circumference and development of obesity.

 

The authors caution that the cross-sectional, observational nature of this study does not allow for conclusions about cause and effect. Estimates of sleep duration were based on self-report data rather than objective measures and may reflect 'time in bed', actual time spent asleep or time people believed they slept. Also, as the study did not distinguish between daytime naps and nighttime sleep, their impact on health could not be assessed separately.

Adolescents stress more with poor sleep

A new study from the University of Alabama at Birmingham indicates that adolescents who experience sleep problems and longer sleep duration are more reactive to stress, which could contribute to academic, behavioral and health issues.

 

Existing studies show that nearly 70 percent of U.S. adolescents do not receive sufficient sleep. It is also known that insufficient sleep and sleep problems contribute to cognitive problems and poor physical health over time, possibly because of disruptions in a key part of the neuroendocrine system that controls reactions to stress and regulates many body processes -- the hypothalamic-pituitary-adrenal axis, or the HPA axis.

 

The relationship between sleep and the HPA axis has been studied in both children and adults; but little is known about this link during adolescence, which is a key period of time, as both sleep and the HPA axis are undergoing significant developmental changes related to puberty.

 

Sylvie Mrug, a psychology professor in UAB's College of Arts and Sciences, and her colleagues from UAB and Arizona State University sought to further explore the relationship between sleep and reactivity to stress, specifically as it relates to HPA-axis activity, in adolescents.

 

The researchers examined two dimensions of sleep -- sleep duration and sleep problems from the perspectives of adolescents and their parents, as well as cortisol levels before and after social stress. The team also looked at how the results varied based on gender.

 

"We chose to look at sleep patterns in urban African-American adolescents, due to information we understood from earlier research in the field," Mrug said. "This particular population is more likely to experience insufficient sleep, and their functioning is more negatively affected by lower sleep quality, so we knew that finding results for this demographic could be especially important."

 

Eighty-four adolescents with an average age of approximately 13 took part in the study. During their visit to the research lab, participating adolescents were given the children's version of a common stress test, called the Trier Social Stress Test, to measure their physiological responses to stress. This test involves speaking and computing mental math problems in front of an audience. Saliva samples were taken from each participant in order to test cortisol levels before and after the stress test.

 

Participants then reported on their bed times and wake times and any sleep problems, such as insomnia, daytime sleepiness and general sleep quality, during a regular week. Parents of the adolescents were asked to report on their children's sleep as well.

 

The adolescents most commonly reported the following sleep problems: the need for multiple reminders to get up in the morning, not having a good night's sleep, feeling tired or sleepy during the day, and not being satisfied with their sleep.

 

The researchers looked at the cortisol levels of the adolescent participants. Cortisol release during and after the stressful lab test was higher for adolescents who reported more sleep problems and longer sleep duration, and whose parents reported longer sleep duration.

 

"The result of higher cortisol levels in adolescents experiencing sleep problems was exactly what we expected to see," Mrug said. "We were, however, surprised that longer sleep duration predicted a stronger cortisol response, because previous studies linked shorter sleep duration with higher cortisol levels. Generally, less sleep is related to poor outcomes, not the other way around. In this case, this unexpected result could be explained by considering that longer sleep duration does not necessarily reflect higher-quality sleep, but instead may serve as another indicator of sleep problems, at least among urban adolescents."

 

The effects of sleep problems on greater cortisol release during stress were stronger in females than in males, suggesting that adolescent girls may be more sensitive to disrupted and poor quality sleep.

 

"Overall, the results of our study confirm what we originally hypothesized -- that sleep problems induce greater response to stress in adolescents," Mrug said. "It's important that we know this, because the enhanced and prolonged activation of the HPA axis in response to stress could contribute to more health problems. The urban African-American youth whom we studied may be particularly negatively affected by poor sleep because they are more likely to experience uncontrollable stress related to community and school violence. We want to do all that we can to understand ways we can help ensure better cognitive, emotional and physical health outcomes for these adolescents."
